Focus: The functional role of long non-coding RNA in human carcinomas


Long non-coding RNAs (lncRNAs) are emerging as new players in the cancer paradigm demonstrating potential roles in both oncogenic and tumor suppressive pathways. These novel genes are frequently aberrantly expressed in a variety of human cancers, however the biological functions of the vast majority remain unknown. Recently, evidence has begun to accumulate describing the molecular mechanisms by which these RNA species function, providing insight into the functional roles they may play in tumorigenesis. In a review article, researchers highlight the emerging functional role of lncRNAs in human cancer.

Les longs ARN non-codants (lncARNs) apparaissent comme des acteurs nouveaux du cancer. Ils démontrent un rôle potentiel tant comme oncogènes que comme suppresseurs de tumeur. Ces gènes récemment identifiés sont souvent exprimés de manière aberrante dans une variété de cancers humains, mais la fonction biologique de la grande majorité d’entre eux  reste inconnue. Récemment, les mécanismes moléculaires par lesquels ces ARNs fonctionnent ont commencé à être éclaircis, offrant un aperçu des rôles fonctionnels qu'ils peuvent jouer dans la tumorigenèse. Dans un article de revue, des chercheurs mettent en évidence le nouveau rôle fonctionnel de lncARNs dans le cancer humain.

Therapeutic class: antineoplastic
Action: Imatinib is a specific inhibitor of the tyrosine kinase domain in abl (the Abelson proto-oncogene), c-kit and PDGFR (platelet-derived growth factor receptor).

Imatinib mesylate is approved (2011) for:


-          Newly diagnosed adult patients with Philadelphia chromosome positive (Ph+) chronic myeloid leukemia (CML) in chronic phase.
-          Patients with Philadelphia chromosome positive (Ph+) chronic myeloid leukemia (CML) in blast crisis, accelerated phase, or in chronic phase after failure of interferon-alpha therapy.
-          Pediatric patients with Ph+ CML in chronic phase who are newly diagnosed or whose disease has recurred after stem cell transplant or who are resistant to interferon-alpha therapy.
-          Adult patients with relapsed or refractory Philadelphia chromosome positive acute lymphoblastic leukemia (ALL).

PANITUMUMAB

Pharmacological class: epidermal growth factor receptor (EGFR) inhibitor
Therapeutic class: antineoplastic
Action: panitumumab is a recombinant, human monoclonal antibody that binds specifically to the epidermal growth factor receptor (EGFR). Overexpression of EGFR is detected in many human cancers, including those of the colon and rectum. When panitumumab binds to EGFR, it competitively inhibits the binding of other ligands for EGFR. This results in inhibition of cell growth, induction of apoptosis, decreased pro-inflammatory cytokine and vascular growth factor production.

Panitumumab is approved (2011):

            - for the treatment of EGFR-expressing, metastatic colorectal carcinoma with disease progression on or following fluoropyrimidine-, oxaliplatin-, and irinotecan-containing chemotherapy regimens.

Focus : Transcriptional consequences of genomic structural aberrations in breast cancer


Using a long-span, paired-end deep sequencing strategy, researchers have comprehensively identified cancer genome rearrangements in eight breast cancer genomes. They show that 40%–54% of these structural genomic rearrangements result in different forms of fusion transcripts and that 44% are potentially translated. They find that single segmental tandem duplication spanning several genes is a major source of the fusion gene transcripts in both cell lines and primary tumors involving adjacent genes placed in the reverse-order position by the duplication event. Certain other structural mutations, however, tend to attenuate gene expression. From these candidate gene fusions, these researchers have found a fusion transcript (RPS6KB1–VMP1) recurrently expressed in ~30% of breast cancers associated with potential clinical consequences. This gene fusion is caused by tandem duplication on 17q23 and appears to be an indicator of local genomic instability altering the expression of oncogenic components such as MIR21 and RPS6KB1.

A l’aide d’une technique spécifique de séquençage (dite « d’extrémités appariées »), des chercheurs ont identifié de manière globale les réarrangements de génome cancéreux dans huit génomes de cancer du sein. Ils montrent que 40%-54% de ces réarrangements génomiques structuraux donnent lieu à différentes formes de transcrits « de fusion » et que 44% de ces transcrits sont potentiellement traduits. Certaines autres mutations structurelles, cependant, ont tendance à atténuer l'expression de gènes. Les chercheurs ont trouvé un transcrit de fusion (RPS6KB1-VMP1) récurrent exprimé dans environ 30% des cancers du sein et associé à des conséquences cliniques potentielles. Le gène de fusion correspondant est produit par duplication en tandem dans le locus 17q23 et semble être un indicateur d'instabilité génomique locale modifiant l'expression d’oncogènes composants tels que MIR21 et RPS6KB1.

NILOTINIB

Pharmacological class : inhibitor of Bcr-Abl tyrosine kinase
Therapeutic class : antineoplastic
Action : nilotinib is an aminopyrimidine-derivative Bcr-Abl tyrosine kinase inhibitor with antineoplastic activity. Designed to overcome imatinib resistance, nilotinib binds to and stabilizes the inactive conformation of the kinase domain of the Abl protein of the Bcr-Abl fusion protein, resulting in the inhibition of the Bcr-Abl-mediated proliferation of Philadelphia chromosome-positive (Ph+) chronic myeloid leukemia (CML) cells. This agent also inhibits the receptor tyrosine kinases platelet-derived growth factor receptor (PDGF-R) and c-kit, a receptor tyrosine kinase mutated and constitutively activated in most gastrointestinal stromal tumors (GISTs). With a binding mode that is energetically more favorable than that of imatinib, nilotinib has been shown to have an approximately 20-fold increased potency in kinase and proliferation assays compared to imatinib.

Nilotinib, used as hydrochloride monohydrate salt, is approved (2011) :
-         for the treatment of adult patients with newly diagnosed Philadelphia chromosome positive chronic myeloid leukemia (Ph+ CML) in chronic phase (CP-CML);
-         for the treatment of chronic phase (CP) and accelerated phase (AP) Philadelphia chromosome positive (Ph+) chronic myelogenous leukemia (CML) in adult patients resistant or intolerant to prior therapy that included imatinib.

Conçu pour surmonter la résistance des cellules tumorales à l'imatinib, le nilotinib se fixe au domaine kinase de la portion Abl de la protéine de fusion Bcr-Abl et stabilise la conformation inactive de ce domaine ; il en résulte une inhibition de la prolifération médiée par Bcr-Abl dans les leucémies myéloides chroniques positives pour le chromosome de Philadelphie (Ph +). Le nilotinib inhibe également la tyrosine kinase du récepteur au « platelet-derived growth factor » (PDGF-R) et celle de c-kit, qui est muté et constitutivement activé dans la plupart des tumeurs stromales gastro-intestinales. Avec un mode de liaison qui est énergétiquement plus favorable que celui de l'imatinib, le nilotinib s’est révélé avoir une efficacité d'environ 20 fois plus élevée que celle de l’imatinib l'imatinib.

En 2011, le nilotinib, utilisé sous forme de chlorhydrate monohydraté, est approuvé:
           
            - pour le traitement des patients adultes atteints de leucémie myéloïde chronique (LMC) positive pour le chromosome de Philadelphie (Ph +) et en phase chronique (LMC-PC);
            - pour le traitement des patients adultes atteints de leucémie myéloïde chronique (LMC) positive pour le chromosome de Philadelphie (Ph +) et en phase chronique (LMC-PC) ou accélérée (LMC-PA), et qui se révèlent résistants ou intolérants à un  traitement antérieur comprenant de l'imatinib.

Focus: Characteristic patterns of structural variations in epithelial cancer genomes – Variations structurales caractéristiques dans les génomes des cancers épithéliaux




Somatic genome rearrangements are thought to play important roles in cancer development. Using a specific sequencing approach, researchers have comprehensively characterized the human genome structural variations (SV) of 15 cancer and two noncancer genomes. Their analyses revealed that most inversions, deletions, and insertions are germ-line SVs, whereas tandem duplications, unpaired inversions, interchromosomal translocations, and complex rearrangements are over-represented among somatic rearrangements in cancer genomes. Researchers have also discovered that large duplications are among the initial rearrangements that trigger genome instability for extensive amplification in epithelial cancers.

Les réarrangements somatiques du génome sont supposés jouer un rôle important dans le développement du cancer. En utilisant une approche de séquençage spécifique, des chercheurs ont globalement caractérisé les variations structurales (VS) de génome humain dans 15 génomes cancéreux et deux génomes non cancéreux. Leurs analyses ont révélé que la plupart des inversions, des suppressions et des insertions sont des VS germinales, tandis que les duplications en tandem, les inversions non appariées, les translocations inter-chromosomiques, et les réarrangements complexes sont surreprésentés parmi les réarrangements somatiques des génomes cancéreux. Les chercheurs ont également découvert que les larges duplications comptent parmi les réarrangements initiaux qui déclenchent l'instabilité du génome menant à des événements d’amplification extensifs observés dans les cancers épithéliaux (trad. Marc Lacroix).

CETUXIMAB


Name : cetuximab
Commercial name : Erbitux
Pharmacologic class : Epidermal growth factor receptor (EGFR) inhibitor
Therapeutic class : Antineoplastic
Action : Binds to EGFR, competitively inhibiting binding of epidermal growth factor and other ligands and blocking phosphorylation and activation of receptor-associated kinases. These actions lead to cell growth inhibition, apoptosis induction, and decreased matrix metalloproteinases and vascular endothelial growth factor.

Cetuximab approved (2011):
-          for treatment of EGFR-expressing metastatic colorectal carcinoma. Cetuximab is used alone in patients intolerant to irinotecan-based chemotherapy or in combination with irinotecan in patients refractory to irinotecan-based therapy
-          for treatment of locally or regionally advanced squamous-cell carcinoma of head and neck, in combination with radiation therapy
-          for recurrent or metastatic squamous-cell carcinoma of head and neck. Cetuximab is used alone in patients for whom platinum-based therapy has failed

Action: le cetuximab se lie à l'EGFR, inhibant de manière concurrentielle la liaison du facteur de croissance épidermique (EGF) et d'autres ligands de l’EGFR. Ceci conduit in fine à l'inhibition de la croissance cellulaire, à l’induction de l'apoptose et une diminution des taux de métalloprotéinases matricielles (enzymes de dégradation) et de facteur de croissance vasculaire endothéliale (impliqué dans l’angiogénèse).

En 2011, le cetuximab est approuvé :
            -pour le traitement du carcinome colorectal métastatique exprimant l'EGFR. Le cetuximab est utilisé seul chez les patients intolérants à la chimiothérapie à base d'irinotécan ou en combinaison avec l'irinotécan chez les patients réfractaires à la thérapie à base d'irinotecan.
            -pour le traitement du carcinome épidermoïde de la tête et du cou au niveau local ou régional avancé, en combinaison avec la radiothérapie
            -pour le traitement du carcinome épidermoïde de la tête et du cou récurrent ou métastatique. Le cetuximab est utilisé seul chez les patients pour lesquels un traitement à base de platine a échoué.

GEFITINIB


Name: gefitinib
Commercial name: Iressa
Pharmacologic class: Epidermal growth factor receptor inhibitor
Therapeutic class: Antineoplastic
Action: unclear. Inhibits tyrosine kinase action, which inhibits cell growth and reproduction. May also inhibit angiogenesis in tumor cells.

Gefitinib approved (2011):
            - for treatment of patients with locally advanced or metastatic non-small-cell lung cancer (NSCLC) after failure of platinum-based and docetaxel chemotherapy

            - pour le traitement de patients ayant un cancer du poumon non à petites cellules (CBNPC) localement avancé ou métastatique, après l'échec d’une chimiothérapie basée sur les composés au platine et au docétaxel.
