Focus: An immunologic portrait of cancer.


            The advent of high-throughput technology challenges the traditional histopathological classification of cancer, and proposes new taxonomies derived from global transcriptional patterns. Although most of these molecular re-classifications did not endure the test of time, they provided bulk of new information that can reframe our understanding of human cancer biology. Here, we focus on an immunologic interpretation of cancer that segregates oncogenic processes independent from their tissue derivation into at least two categories of which one bears the footprints of immune activation. Several observations describe a cancer phenotype where the expression of interferon stimulated genes and immune effector mechanisms reflect patterns commonly observed during the inflammatory response against pathogens, which leads to elimination of infected cells. As these signatures are observed in growing cancers, they are not sufficient to entirely clear the organism of neoplastic cells but they sustain, as in chronic infections, a self-perpetuating inflammatory process. Yet, several studies determined an association between this inflammatory status and a favorable natural history of the disease or a better responsiveness to cancer immune therapy. Moreover, these signatures overlap with those observed during immune-mediated cancer rejection and, more broadly, immune-mediated tissue-specific destruction in other immune pathologies. Thus, a discussion concerning this cancer phenotype is warranted as it remains unknown why it occurs in immune competent hosts. It also remains uncertain whether a genetically determined response of the host to its own cancer, the genetic makeup of the neoplastic process or a combination of both drives the inflammatory process. Here we reflect on commonalities and discrepancies among studies and on the genetic or somatic conditions that may cause this schism in cancer behavior.

            L'avènement de technologies « à haut débit » défie la classification traditionnelle histopathologique du cancer, et propose de nouvelles taxonomies issues de modèles globaux de transcription. Bien que la plupart de ces re-classifications moléculaires n’aient pas supporté l'épreuve du temps, ils ont fourni de nouvelles informations en vrac qui peuvent recadrer notre compréhension de la biologie du cancer humain. Ici, nous nous concentrons sur une interprétation immunologique du cancer qui sépare les processus oncogéniques, indépendamment de leur tissu de dérivation, dans au moins deux catégories dont une porte l'empreinte de l'activation immunitaire. Plusieurs observations décrivent un phénotype du cancer, où l'expression de gènes stimulés par l'interféron et des mécanismes effecteurs immunitaires reflètent des tendances observées couramment lors de la réaction inflammatoire contre les agents pathogènes, ce qui conduit à l'élimination des cellules infectées. Focus: The nuclear epidermal growth factor receptor signaling network and its role in cancer

The epidermal growth factor receptor (EGFR) is a member of the EGFR family of receptor tyrosine kinases (RTKs). EGFR activation via ligand binding results in signaling through various pathways ultimately resulting in cellular proliferation, survival, angiogenesis, invasion, and metastasis. Aberrant expression or activity of EGFR has been strongly linked to the etiology of several human epithelial cancers including but not limited to head and neck squamous cell carcinoma (HNSCC), non-small cell lung cancer (NSCLC), colorectal cancer (CRC), breast cancer, pancreatic cancer, and brain cancer. Thus intense efforts have been made to inhibit the activity of EGFR by designing antibodies against the ligand binding domains (cetuximab and panitumumab) or small molecules against the tyrosine kinase domain (erlotinib, gefitinib, and lapatinib). Although targeting membrane-bound EGFR has shown benefit, a new and emerging role for EGFR is now being elucidated. In this review we will summarize the current knowledge of the nuclear EGFR signaling network, including how it is trafficked to the nucleus, the functions it serves in the nucleus, and how these functions impact cancer progression, survival, and response to chemotherapeutics.

Le récepteur de facteur de croissance épidermique (EGFR) est un membre de la famille « EGFR » de récepteurs à tyrosine kinases (RTK). Son activation par un ligand entraîne une signalisation par diverses voies conduisant à la prolifération cellulaire, à la survie, à l'angiogénèse, à l'invasion et aux métastases. L'expression ou l'activité aberrante de l'EGFR sont fortement liées à l'étiologie de cancers épithéliaux humains, parmi lesquels ceux touchant  la tête et le cou,  le poumon, le colon, le sein, le pancréas et le cerveau. Des efforts intenses ont été développés pour inhiber l'activité de l'EGFR en concevant des anticorps contre les domaines de liaison au ligand (cetuximab et panitumumab) ou de petites molécules contre le domaine tyrosine kinase (erlotinib, gefitinib, et le lapatinib).
