NELARABINE

Pharmacological class: nucleoside analogue

Therapeutic class: antineoplastic

Action: nelarabine is a pro-drug of the deoxyguanosine analogue 9-β-D-arabinofuranosylguanine (ara-G). Nelarabine is demethylated by adenosine deaminase to ara-G, which is subsequently converted to the active 5'-triphosphate, ara-GTP. The leukemic blasts gather the active form allowing for the incorporation into the deoxyribonucleic acid (DNA) which results in inhibition of DNA synthesis and eventual cell death.

In 2011, nelarabine is approved:

            ● for the treatment of patients with T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma (T-LBL) whose disease has not responded to or has relapsed following treatment with at least two chemotherapy regimens.

Action: la nélarabine est une pro-drogue de l'analogue de la désoxyguanosine 9-β-D-arabinofuranosylguanine (ara-G). La nélarabine est déméthylée par l'adénosine désaminase en ara-G, qui est ensuite convertie en forme active 5'-triphosphate, ara-GTP. Les blastes leucémiques captent la forme active, permettant son incorporation dans l’ADN, ce qui conduit à l'inhibition de la synthèse d'ADN et à la mort cellulaire.

 En 2011, la nélarabine est approuvée:

             ● pour le traitement de patients avec leucémie aiguë lymphoblastique à cellules T (LAL-T) ou lymphome lymphoblastique à cellules T (LLB-T) dont la maladie n'a pas répondu ou qui ont rechuté après un traitement par au moins deux protocoles chimiothérapeutiques.
