RITUXIMAB

Name: rituximab

Commercial name: Rituxan

Pharmacologic class: Murine/human monoclonal antibody

Therapeutic class: antineoplastic

Action: Binds to CD20 antigen on malignant B lymphocytes; recruits immune effector functions to mediate B-cell lysis (possibly through complement-dependent cytotoxicity and antibody-dependent cell-mediated cytotoxicity)

In 2011, rituximab is approved:

           

            ● for maintenance therapy in patients with previously untreated follicular CD-20 positive B-cell non-Hodgkin lymphoma who achieve a response to rituximab in combination with chemotherapy;

            ● in combination with fludarabine and cyclophosphamide (FC), for the treatment of both previously untreated and previously treated patients with chronic lymphocytic leukemia (CLL);

            ● for use in the first-line treatment of patients with low grade or follicular B-cell, CD20-positive non-Hodgkin’s lymphoma. One approval was for the use of rituximab combined with CVP chemotherapy (cyclophosphamide, vincristine, and prednisone); the second is for use of rituximab following CVP chemotherapy;

            ● for use in the first-line treatment of patients with diffuse large B-cell, CD20-positive, non-Hodgkin’s lymphoma in combination with cyclophosphamide, doxorubicin, vincristine and prednisone (CHOP) or other anthracycline-based chemotherapy regimens.

Action: le rituximab se lie à l'antigène CD20 sur les lymphocytes B malins; il recrute les fonctions effectrices du système immunitaire, entraînant la lyse des cellules B.

En 2011, le rituximab est approuvé (2011):

 ● dans le traitement d'entretien des patients ayant un lymphome non hodgkinien folliculaire à cellules B CD-20-positives, et qui répondent au rituximab en combinaison avec de la chimiothérapie ;

 ● en association avec la fludarabine et le cyclophosphamide dans le traitement des patients atteints de leucémie lymphoïde chronique (LLC) préalablement traitée ou non ;

● dans le traitement de première ligne de patients atteints de lymphome non-hodgkinien de bas grade ou folliculaire à cellules B CD20-positives. L’utilisation du rituximab est approuvée aussi bien en association avec une chimiothérapie « CVP » (cyclophosphamide, vincristine et prednisone) qu’après cette chimiothérapie CVP ;

 ● dans le traitement de première ligne de patients atteints de lymphome non-hodgkinien diffus à grandes cellules B CD20-positives, en association avec la combinaison cyclophosphamide, doxorubicine, vincristine et prednisone (chimiothérapie « CHOP ») ou avec d'autres protocoles de chimiothérapie à base d'anthracyclines.
