OFATUMUMAB


Name: ofatumumab
Commercial name: Arzerra
Pharmacologic class: Cytolytic monoclonal antibody (CD20-directed)
Therapeutic class: antineoplastic
Action: ofatumumab is a unique human IgG1 monoclonal antibody that targets a distinct small loop epitope on the CD20 molecule. It binds CD20 relatively tightly and has a reduced off rate compared to rituximab. The mechanisms of actions of ofatumumab include complement dependent cytotoxicity (CDC) and antibody dependent cell-mediated cytotoxicity (ADCC). Ofatumumab has the ability to lyse CD20-expressing cells in vitro more efficiently than rituximab, especially in cells with a relatively low CD20 expression (such as CLL) and in cells expressing high levels of complement-regulatory proteins

In 2011, ofatumumab is approved:

            ● for patients with chronic lymphocytic leukemia whose cancer is no longer being controlled by other forms of chemotherapy (fludarabine, alemtuzumab).

Action: romidepsin is a histone deacetylase (HDAC) inhibitor. HDACs catalyze the removal of acetyl groups from acetylated lysine residues in histones, resulting in the modulation of gene expression. HDACs also deacetylate non-histone proteins, such as transcription factors.

In 2011, romidepsin is approved:

            ● for the treatment of cutaneous T-cell lymphoma (CTCL) in patients who have received at least one prior systemic therapy.

Action: cabazitaxel is a microtubule inhibitor. Cabazitaxel binds to tubulin and promotes its assembly into microtubules while simultaneously inhibiting disassembly. This leads to the stabilization of microtubules, which results in the inhibition of mitotic and interphase cellular functions.

In 2011, cabazitaxel is approved:

            ● for use in combination with prednisone for treatment of metastatic hormone refractory prostate cancer previously treated with a docetaxel-containing regimen.

EVEROLIMUS

Pharmacologic class: small mTOR inhibitor
Therapeutic class: antineoplastic
Action: everolimus is an inhibitor of mTORC1 (mammalian target of rapamycin complex 1). mTORC1 plays an essential role in protein synthesis downstream of the P13K/AKT pathway, which is deregulated in many human cancers. Everolimus has been shown to reduce cell proliferation, glycolysis and angiogenesis in solid tumors in vivo. It also possesses immunosuppressive activity.

In 2011, everolimus is approved:

            ● for progressive neuroendocrine tumors of pancreatic origin (PNET) in unresectable, locally advanced or metastatic disease.
            ● for subependymal giant cell astrocytoma (SEGA) associated with tuberous sclerosis (TS) that cannot be surgically removed.
            ● for advanced renal cell carcinoma after failure of treatment with sunitinib or sorafenib.

ERIBULIN

Pharmacologic class: non-taxane microtubule dynamics inhibitor
Therapeutic class: antineoplastic
Action: eribulin mesylate is a microtubule inhibitor. It prevents the microtubules from growing but does not affect the shortening of the microtubules in mitosis, making the tubulin nonfunctional. This causes the cell to stop in the G2/M phase of the cell cycle, which disrupts the mitotic spindles and causes cell death as a result of prolonged mitotic blockage.


In 2011, eribulin mesylate is approved:

            ● for the treatment of patients with metastatic breast cancer who have previously received an anthracycline and a taxane in either the adjuvant or metastatic setting and at least two chemotherapeutic regimens for the treatment of metastatic disease.

Focus: the management of cancer pain



The experience of pain in cancer is widely accepted as a major threat to quality of life, and the relief of pain has emerged as a priority in oncology care. Pain is associated with both the disease as well as treatment, and management is essential from the onset of early disease through long-term survivorship or end-of-life care. Effective relief of pain is contingent upon a comprehensive assessment to identify physical, psychological, social, and spiritual aspects and as a foundation for multidisciplinary interventions. Fortunately, advances in pain treatment and in the field of palliative care have provided effective treatments encompassing pharmacological, cognitive-behavioral, and other approaches. The field of palliative care has emphasized that attention to symptoms such as pain is integral to quality cancer care.

L'expérience de la douleur dans le cancer est largement considérée comme une menace majeure pour la qualité de vie, et le soulagement de la douleur est vu comme une priorité dans les soins en oncologie. La douleur est associée à la fois à la maladie et au traitement et la gestion est essentielle dès le début de la maladie. Le soulagement efficace de la douleur est subordonné à une évaluation complète identifiant les aspects physiques, psychologiques, sociaux et spirituels, servant de base à une approche multidisciplinaire. Il est heureux que les progrès dans le traitement de la douleur et dans le domaine des soins palliatifs aient débouché sur des traitements efficaces qui englobent des approches pharmacologiques, cognitivo-comportementale, et d'autres. Il est aujourd’hui clair que l'attention portée à des symptômes tels que la douleur fait partie intégrante de soins de qualité.

RITUXIMAB

Action: Binds to CD20 antigen on malignant B lymphocytes; recruits immune effector functions to mediate B-cell lysis (possibly through complement-dependent cytotoxicity and antibody-dependent cell-mediated cytotoxicity)

In 2011, rituximab is approved:
           
            ● for maintenance therapy in patients with previously untreated follicular CD-20 positive B-cell non-Hodgkin lymphoma who achieve a response to rituximab in combination with chemotherapy;
            ● in combination with fludarabine and cyclophosphamide (FC), for the treatment of both previously untreated and previously treated patients with chronic lymphocytic leukemia (CLL);
            ● for use in the first-line treatment of patients with low grade or follicular B-cell, CD20-positive non-Hodgkin’s lymphoma. Ibritumomab tiuxetan is combined with a radioactive substance and given with rituximab, as a type of radioimmunotherapy.

Focus: cancer stem cell subsets and their relationships


Emerging evidence suggests that cancer stem cells account for the initiation and progression of cancer. While many types of cancer stem cells with specific markers have been isolated and identified, a variety of differences among them began to be appreciated. Cancer stem cells are hierarchical populations that consist of precancerous stem cells, primary cancer stem cells, migrating cancer stem cells and chemoradioresistant cancer stem cells, playing different roles in cancer initiation and progression. Researchers propose a new concept "horizontal hierarchy of cancer stem cells" to distinguish them from vertical hierarchy cancer stem cells, cancer transient-amplifying cells and cancer differentiated cells, and summarize our current understanding of these subsets of cancer stem cells with the aim to open up novel therapeutic strategies for cancer based on this understanding.

De récentes données suggèrent que les cellules souches cancéreuses rendent compte de l'initiation et de la progression du cancer. Les cellules souches cancéreuses sont des populations hiérarchiques qui se composent de cellules souches précancéreuses, cellules souches de cancer primaire, cellules souches de cancer métastatique et cellules souches de cancer chemoradioresistant. Ces populations jouent des rôles différents dans l'initiation et la progression du cancer. Des chercheurs proposent un nouveau concept  de « hiérarchie horizontale de cellules souches cancéreuses » pour les distinguer de la «  hiérarchie verticale », des cellules cancéreuses à amplification transitoire et des cellules cancéreuses différenciées. Ils résument notre compréhension actuelle de ces sous-ensembles de cellules souches du cancer dans le but d’ouvrir de nouvelles stratégies thérapeutiques pour le cancer.

Action: A monoclonal antibody directed against cytotoxic T-lymphocyte-associated antigen-4 (CTLA4), an antigen that is expressed on activated T-cells and exhibits affinity for B7 co-stimulatory molecules. By binding CTLA4, ipilimumab enhances T-cell activation and blocks B7-1 and B7-2 T-cell co-stimulatory pathways.

            ● for unresectable or metastatic melanoma

VANDETANIB

Name: vandetanib

Pharmacological class: tyrosine kinase inhibitor
Therapeutic class: antineoplastic
Action: Vandetanib is an orally bioavailable 4-anilinoquinazoline. Vandetanib selectively inhibits the tyrosine kinase activity of vascular endothelial growth factor receptor 2 (VEGFR2), thereby blocking VEGF-stimulated endothelial cell proliferation and migration and reducing tumor vessel permeability. This agent also blocks the tyrosine kinase activity of epidermal growth factor receptor (EGFR), a receptor tyrosine kinase that mediates tumor cell proliferation and migration and angiogenesis.


In 2011, vandetanib is approved:

            ● for unresectable, locally advanced, or metastatic medullary thyroid cancer.

Action: le vandetanib est une 4-anilinoquinazoline administrée par voie orale. Le vandetanib inhibe  sélectivement l'activité tyrosine kinase du récepteur 2 au facteur de croissance endothelial vasculaire (« vascular endothelial growth factor receptor 2 » or VEGFR2), ce dont il résulte le blocage de la prolifération des cellules endothéliales et de la migration cellulaire VEGF-induites et la réduction de la perméabilité des vaisseaux tumoraux. Des gouttelettes pour détecter l'ADN tumoral
Dans un avenir proche, détecter un cancer sera peut-être possible par une simple analyse de sang ou d'urine. En effet, des biologistes du CNRS, de l'Inserm et des universités Paris Descartes et de Strasbourg viennent de mettre au point une technique capable de déceler les infimes traces d'ADN tumoral présentes dans les fluides biologiques de patients atteints d'un cancer.
